* Collect the Admission Form from Office or download the Admission Form from our website www.alliedhealthindia.com.
* Attach one copy coloured passport sized photograph in the application form and submit extra 4 copies pp photo.
* As residential proof, attach photocopy of Voter Card / Aadhar Card / Ration Card / Passport / Certificate from Panchayat /Councillor.
* Attested photocopies of Admit Card , Mark Sheet and Certificate of Madhyamik / ICSE / CBSE / NIOS or equivalent.
* Submit the Form within stipulated date with required Admission Fee.
MODE OF PAYMENT : Payment must be done by CASH / Bank Details given by the Institute.
